MEMBERSHIP ENROLLMENT FORM AND FEE

Membership Fee - There is an annual Membership Fee of $25.00, per child.  Upon receipt of the membership fee, you will receive your official CSU T-Shirt and membership certificate.  Please allow 4-6 weeks for delivery of membership items.

Membership Enrollment Form - To join the Chocolate State University^TM Membership Club, complete the Membership Enrollment Form online and print it. (Don’t forget to enclose your annual membership fee of $25.00.)  At this time, we accept checks or money orders.  Please make your check or money order payable to FOUR C’S PRODUCTIONS.
